How can I prepare for my first appointment? 

When you come in for your appointment, there are a couple of things you will want to bring with you, including a list of any prescription medications you are taking and your dental insurance card. So be sure to get these together before your visit. 

Also, if possible, fill out the two new patient forms on our website prior to your appointment. If you are unable to do so, then please plan to arrive at least 10 to 15 minutes early so that you can complete them in our office.
